Child must be 3 or 4 years old on or before September 1, 2022 to be eligible for this program.
 
Documents needed to determine eligibility for Head Start:
Proof of age (birth certificate, hospital birth record, passport)
Proof Income for the previous calendar year or the last 12 months
PRE-K
Child must be 4 years old on or before September 1, 2022 to be eligible for this Program
Documents needed to determine eligibility for Pre-K are:
Proof of age (birth certificate, hospital birth record, passport)
Copy of child’s social security card (if applicable)
Proof of Residency (Pre-K Only)
 
 Requested documents for all programs:
Child’s immunization record (Form 3231)
Copy of child’s medical insurance (if applicable)
